Record: ADR-129, all of it. The _async form ends with <package>_op** op (or NULL); <package>_cancel cancels the task at its next pause point with cleanup run; done is called exactly once, E_CANCELLED (-7) when the cancellation came first; <package>_op_free after done. A function producing many results takes fn(item) -> bool sync, whose false stops it, and the next item is produced only after the callback returned; a returned list of text or handles is refused naming that shape.

Order (§5): the ticket, cancel, op_free and the code; the bool callback row and the refusal message; a streamed file and a cancelled fetch in the C example.
